If you are seeking financial assistance, medical assistance, or would like to be considered for one of our Programs, you MUST first provide - VIA EMAIL -  a copy of your VA CARD or a copy of your DD-214, or both. Take a picture of these documents, and email them to us at the above email address and type Veteran Assistance in the subject line. 

Tell us EXACTLY what it is you need, why you need it, and how we can help you. PLEASE BE SPECIFIC. You must provide an EXACT DOLLAR AMOUNT if you are requesting financial assistance. This will help our staff serve you quicker and more efficiently. If you do not provide this requested information, we will not be able to contact you.

When we have received your VA CARD and/or your DD-214 you will be sent, or emailed, a comprehensive, PRELIMINARY VETERAN INTAKE FORM. Please complete it, sign it and return it to us via email or regular mail.
